Fees and Policies

New Clients

Please arrive at least 20 minutes early for your first visit to fill out necessary paperwork, which includes a thorough health history. If you prefer you can download the forms here to save time.

Contraindications for treatment

Please note that acupuncture is contraindicated if you are altered by alcohol or drugs. You may also wish to delay treatment if you are excessively fatigued or overly hungry until you have had proper rest and nourishment. I reserve the right to refuse treatment to anyone who is deemed ineligible for such treatment.

Fees

Initial Consultation: $150.00
Regular session: $125.00
Medicinal Herb Consult: $75.00 Initial visit. $25.00 follow-up visit and/or formula modification.
Medicinal Herb Formula: Herb formulas vary in price and depend on whether your prescription utilizes raw herbs, granules or tablets. Typical weekly herb cost is about $20.
Note: If you are coming for regular session and choose to use herbs as well, the initial herb consult fee is $25.00 and follow-up fee and formula modification is $15.00.

Payment Method

Full payment is due at time of service by cash or check unless other arrangements have been previously made.

Insurance

While I do not accept insurance as payment for services, many providers offer out-of-network coverage for acupuncture and alternative health care. I can provide you with an appropriately documented statement to use for reimbursement from your insurance provider.

Cancellation Policy

If you need to cancel your appointment for any reason, the courtesy of a 24 hour advance notice is requested. Cancellations made less than 24 hours in advance are assessed at full fees.

Confidentiality

Client confidentiality is strictly observed at all times. I do not release records or client information except as approved by an authorized request source as required by law.
